Principal care management
Focused support for a complex condition.
Principal care management centers on one serious, complex chronic condition that requires ongoing coordination. Through a participating practice, support follows the plan of the clinician managing that condition.

Before you begin
A clear first step.
Ask about the fit
Your clinician reviews whether focused care management is appropriate.
Clarify who is responsible
Understand which clinician manages the condition and how this work connects with other care teams.
Review the arrangement
Discuss consent, expected costs, and how the team will follow up with you.
Questions are welcome
A little more clarity.

Can I have other health conditions?
Yes. The focus of this program is one complex condition, but you may have other conditions. Your team considers how the care plan connects with your overall care.
Is this the same as chronic care management?
CCM addresses multiple chronic conditions together. PCM focuses on a single complex condition. Your clinician explains the appropriate service and how it relates to care you already receive.
