Use the clinical context.
A medication question makes more sense when the person listening understands the chart, the care plan, and what has changed.
About Torch
Torch was built around a simple need: patients still have questions, medication concerns, and follow-up needs after they leave the doctor’s office.
Our pharmacists and care coordinators work alongside the practice to help patients understand their care, identify concerns, and bring useful information back to the people directing treatment.
The practice remains the patient’s clinical home. Torch adds time and attention to the work between visits.
A medication question makes more sense when the person listening understands the chart, the care plan, and what has changed.
We bring relevant findings and recommendations to the practice, then help communicate and follow through on the plan.
Our team is based in Alabama. Familiar people and clear points of contact help patients and practices build a working relationship.
