Annual wellness visits

A visit to plan for your health.

An annual wellness visit is a chance to review your health risks and create or update a personal prevention plan with your practice. It is a planning visit, different from a routine annual physical.

Ask your care team

What care looks like

Understand the support.

Review your health history

Discuss your health, medicines, other clinicians, and questions about daily activities or safety.

Identify what is due

Your practice reviews appropriate screenings, vaccinations, and other preventive care.

Leave with a plan

A written prevention plan helps you understand recommended next steps and discuss them with your treating team.

Before you begin

A clear first step.

  1. Confirm the visit type

Ask your practice whether you are due for an annual wellness visit.

  2. Get your information ready

Bring or share your medication list, health history, other providers, and questions.

  3. Review the next steps

Discuss the prevention plan and clarify any separate appointments or services you need.

Questions are welcome

A little more clarity.

For more about the program, read Medicare’s annual wellness visit guide.

Is an annual wellness visit a physical?

No. The purpose is to assess health risks and plan preventive care. A physical exam or treatment of a new concern may be a separate service.

Will I owe anything?

For eligible Medicare wellness visits, you pay nothing if the provider accepts assignment. Additional tests or services at the same appointment may have a separate cost. Ask the practice what is planned.

How often can I have one?

Medicare generally covers a wellness visit once every 12 months when eligibility requirements are met. Your practice checks your visit history and coverage.
