Advanced primary care management
One care plan. People helping it work.
Advanced primary care management brings ongoing care coordination through a practice that serves as your main point of care. Torch can support that practice with medication review, communication, and follow-through.
Ask your care teamWhat care looks like
Understand the support.
A shared picture of your care
Organize your health goals, conditions, medications, and treatment plan so the team can understand what needs attention.
Help across appointments
Coordinate information from specialists, follow-up visits, and changes after a hospital stay with your primary care team.
Follow-up that fits your needs
Discuss medication concerns and barriers to your plan. Your treating practice reviews recommendations and makes clinical decisions.
Before you begin
A clear first step.
Check with your primary care practice
Ask whether the practice offers APCM and how Torch fits into its care team.
Discuss your needs and costs
Review your current care, other care management services, expected responsibility, and the consent agreement.
Know whom to contact
Learn how to reach your practice and care team, including the practice’s arrangement for help outside office hours.
Questions are welcome
A little more clarity.
For more about the program, read Medicare’s advanced primary care management guide.
Is this just for people with several conditions?
The program supports patients with different levels of care needs. Your practice considers your situation and the program requirements rather than relying on a condition count alone.
How is it different from chronic care management?
APCM combines several types of ongoing support through your main primary care practice. CCM focuses on managing multiple chronic conditions. Your practice explains which program fits and how it relates to services you already receive.
Can I choose whether to participate?
Yes. Your provider explains the program and documents your consent before services begin. You can ask questions or discuss stopping participation with the practice.
